What if The Loop guessed wrong and the PR solved the wrong problem?
Find the gap between what the agent had and what it needed, then add that missing signal to what context-loop checks, so the next pass catches it.
Read the task file and its ## Loop notes block, which records what was decided and why. Common gaps are a convention that lives only in your head, a task that read as complete but skipped an edge case, and a stale assumption inferred from an exceptional pattern.
Patching the one task does not stop the next one. Write the missing signal into a conventions doc or the rubric where the next scoring pass will read it.
